Sublingual Immunotherapy Tablets Relieve Symptoms in Adults with Allergic Rhinitis: A Meta-analysis of Randomized Clinical Trials被引量:9

Background: Sublingual immunotherapy (SLIT) has been proven to be effective against house dust mite-induced allergic rhinitis. However, the efficacy in adults with allergic rhinitis has never been reported on SLIT tablets. The current meta-analysis aimed to illustrate the differentiated efficacy of SLIT tablets on allergic rhinitis. Methods: Our systematic review and meta-analysis were performed on allergic rhinitis patients and aimed to summarize those randomized controlled studies (RCTs). PubMed, EMBASE, Cochrane library, and MEDLINE were screened for associated articles. We included RCTs on allergic rhinitis patients undergoing SLIT therapy and reporting outcornes on symptom relief and serum-specific lgE levels. The effect of SLIT tablets on the Rhinitis Quality Life Questionnaire Score (RQLQ), Rhinitis Total Symptom Score (RTSS), and serum-specific IgE levels was evaluated using RevMan 5.3. Results: Seven studies were included, with 2723 patients identified. All of the studies were RCT. The included seven studies were all conducted on adults. Among the included seven articles, five researches administered patients with SLIT tablets and were eligible for meta-analysis of RTSS, consisting of 1490 patients. Overall, RTSS was significantly reduced in the SLIT tablet group compared with that in the placebo group (standard mean difference = -0.33, 95% confidence interval [-0.54, -0.13], P 〈 0.01 ). There was no significant difference in specific lgE levels between SLIT and placebo patients. Conclusions: SLIT tablets effectively relieve rhinitis symptoms in adults with allergic rhinitis. Nevertheless, the current evidence may be limited due to sample size and the heterogeneity between studies. Large sample size and multiple center RCTs on the efficacy of different formulations of SLIT drugs are still needed to provide further evidence and a more precise recommendation.

Comparison and Evaluation of Several Dermatophagoides Pteronyssinus Allergen Extracts for Skin Prick Test被引量:1

Objective To evaluate the significance of several Dermatophagoides pteronyssinus allergen extracts for skin prick test(SPT) in patients allergic to Dermatophagoides pteronyssinus.Methods Two hundred and nineteen patients enrolled in Peking Union Medical College Hospital underwent SPT and serum specific IgE assay to detect the Dermatophagoides pteronyssinus allergen.Three kinds of house dust mite allergen extracts were used for SPT,including the Dermatophagoides pteronyssinus extract prepared by our laboratory(group A),standardized Dermatophagoides pteronyssinus extract(group B),and mixed extracts of Dermatophagoides pteronyssinus and Dermatophagoides farinae(group C).Human serum specific IgE result was regarded as the reference standard for diagnosis of Dermatophagoides pteronyssinus allergy.The receiver operating characteristic(ROC) curve was used to evaluate the diagnostic performance of SPT with the extracts of three groups.Results SPT results showed that the median wheal diameter of group A,group B,and group C was 0.43,0.35,and 0.28 cm,respectively,with significant difference among three groups(P<0.05).The difference was significant between group A and B(P<0.01) as well as group A and C(P<0.01),but not between group B and C(P>0.05).There was no local urticaria or systemic allergic reactions following the procedure of SPT.Local reaction was observed in 5 patients and delayed reaction was in 2 patients of group A.As for group B and C,local reaction occurred in 3 cases and delayed reaction in 2 cases in each group.The area under ROC curve of SPT with extract in group A,group B,and group C was 0.765,0.801,and 0.782,respectively.Based on the detection results of serum specific IgE,the sensitivity of SPT in diagnosis of Dermato-phagoides pteronyssinus allergy with extract of group A,group B,and group C was 92.4%,87.0%,and 81.5%,and the specificity was 60.6%,73.2%,and 74.8%,respectively.Conclusion The Dermatophagoides pteronyssinus extract for SPT prepared by our laboratory offers good sensitivity and specificity co

Comparison of Three Methods of Protein Extraction from Dermatophagoides Pteronyssinus for Two-dimensional Electrophoresis被引量:1

Obiective To explore an effective method of Dermatophagoides pteronyssinus protein extraction suitable for two-dimensional electrophoresis (2-DE) analysis. Methods The extracts of Dermatophagoides pteronyssinus were prepared with Coca's solution, lysis buffer of 2-DE, and Trizol reagent, respectively. Bicinchoninic acid (BCA) assay was used to determine the total protein concentration of the samples. The efficiency of different protein extraction methods were evaluated with 2-DE analysis. Results The concentrations of extracted protein by methods of Coca's solution, lysis buffer, and Trizol reagent were 0.63 g/L, 0.90 g/L, and 0.80 g/L, respectively. The 2-DE analysis results showed that some protein spots in low molecular weight (LMW) range could be detected with the Coca's solution method. With the lysis buffer of 2-DE method, more protein spots in LMW range could be detected, while the medium molecular weight (MMW) protein spots were absent. Several MMW protein spots (174-178 kD and 133 kD) and more LMW protein spots were detected withTrizol reagent method. Conclusions Among Coca's solution, lysis buffer of 2-DE, and Trizol reagent, the concentration of extracted protein of Dermatophagoides pteronyssinus by lysis buffer of 2-DE is the highest. However, most protein components of Dermatophagoides pteronyssinus purified mite bodies can be extracted by Trizol reagent, which may generally reflect the whole profile of Dermatophagoides pteronyssinus allergens.

几种户尘螨过敏原点刺液的临床评价被引量:1

目的评价户尘螨过敏原点刺皮试液用于诊断和筛查尘螨过敏的意义。方法对219例患者进行户尘螨过敏原点刺试验和户尘螨sIgE检测。点刺试验所用点刺液为本课题组研制的点刺液(A组)、ALK公司的户尘螨点刺液(B组)和ALK公司的混合螨(户尘螨和粉尘螨)点刺液(C组),以sIgE为诊断标准。统计分析采用SPSS11.0软件,对3组户尘螨过敏原点刺试验进行ROC曲线评价。结果A、B和C组风团平均直径中位数分别为0.43cm、0.35cm和0.28cm。所有入选者点刺试验后1h,A组有5例点刺试验后出现局部反应,2h左右自行缓解;有2例出现迟发反应,需1d左右才消退;B组和C组各有3例点刺试验后出现局部反应,B组和C组各有2例有迟发反应。所有患者均无局部皮疹、全身性不良反应。3组户尘螨过敏原点刺试验的ROC曲线下面积分别为0.765、0.801和0.782,说明户尘螨过敏原点刺皮试液对诊断户尘螨过敏具有较高准确性。以户尘螨sIgE为标准的点刺试验灵敏度和特异度:A、B和C组点刺液的灵敏度分别为92.4%、87.0%和81.5%,特异度分别为60.6%、73.2%和74.8%。结论户尘螨点刺液可作为尘螨过敏的诊断试剂,有良好的灵敏度和特异度,具有临床筛查和诊断意义。

北京地区尘螨过敏患者家庭螨类调查被引量:8

目的调查北京地区尘螨过敏患者家庭螨类种类和比例,为北京地区尘螨防治提供理论依据。方法以北京协和医院就诊的38例北京地区尘螨过敏患者为研究对象,于2008年12月至2010年1月人户吸尘采集其所在家庭室内尘土样本,采用漂浮法收集尘土中的螨,分离制片,显微镜下分类计数并绘图。结果 38个家庭共采集样本345份,螨检出率为64.6%;经分离鉴定得出蜱螨亚纲3目12科15属22种,其中丝泊尘螨为我国致敏螨类一新纪录种;昆虫纲啮虫目1科1属2种。在分析的1798个标本中,主要致敏螨种前3位的依次为粉尘螨(62.46%),户尘螨(21.47%)和丝泊螨(5.28%)。对不同采集地点的调查结果表明,平房(91.1%)样本阳性率高于楼房(59.5%)。对不同生活环境螨密度的数据分析表明,枕头的平均螨密度最高,达到282只螨/克尘土,其次为床褥(120只螨/克尘土)和沙发(115只螨/克尘土)。平均螨密度在9至10月达最高峰,5至7月和12月至次年1月也出现2个高峰,3月和11月密度最低。结论本研究首次发现并报道了北京地区存在的丝泊尘螨,亦属于亚洲首次报道。北京地区室内优势螨种为粉尘螨,其次为屋尘螨和丝泊尘螨,平均螨密度在9至10月达最高峰。

北京协和医院住院患者严重过敏反应的年代特点分析

回顾性分析1990年至今住院患者严重过敏反应的发生率,42例严重过敏反应患者的一般情况、临床表现、病因,2005年前后严重过敏反应发生率、患者性别年龄差异、临床表现和病因的变化。患者平均年龄(39±16)岁,男:女=1.0:2.2,2005年后严重过敏反应的例数较前明显增加(30例与12例);2005年后发生严重过敏反应的女性明显多于2005年前(24例与5例);2005年后出现神经系统、循环系统症状及血压下降的比例明显低于2005年前(P〈0.01、〈0.05、〈0.01);2005年后手术中发生严重过敏反应的构成比较2005年前明显增加(43.3%与1/12);引起严重过敏反应的最主要病因为药物(35/42),药物中最常见的是抗生素(14/35)。

甘油对户尘螨过敏原点刺液急性毒性实验的影响

摘要:目的探讨甘油对户尘螨过敏原点刺液急性毒性实验的影响。方法按1:10(W/V)比例制备户尘螨过敏原提取液原液。户尘螨过敏原点刺液急性毒性实验:取5~7周龄BALB/c小鼠35只,分成5组,每组7只。在每组小鼠腹腔内分别注射0.5 ml不同液体,具体如下:A组含0.4%苯酚的户尘螨过敏原提取液原液(3 ml)+甘油(3 ml);B组含户尘螨过敏原提取液原液(3 ml)+生理盐水(3 ml);C组含甘油(3 ml)+生理盐水(3 ml);D组含0.4%苯酚的生理盐水(6 ml);E组含生理盐水(6 ml)。结果A组和C组内所有小鼠均在注射后10分钟左右出现活动减少、竖尾,15-18分钟全身躁动、抖动、打滚,20~25分钟小鼠死亡。B、D和E组小鼠腹腔注射后活动正常,观察7天均存活。A组和C组死亡的小鼠立即进行病理解剖,结果显示腹腔内大量腹水、肠管水肿;肺组织高度淤血,肺泡壁增厚增宽,肺泡壁及肺间质毛细血管高度扩张,肺间质可见多量淋巴细胞浸润。小肠壁疏松、水肿,黏膜上皮变性、坏死、脱落。肝、脾、肾组织未见特殊改变。B、D和E组小鼠的肺、肝、脾、肾组织未见特殊改变。结论小鼠户尘螨过敏原点刺液急性毒性实验应该使用不含有50%甘油的户尘螨过敏原点刺液原液。
