目的采用情绪评估量表和任务态功能磁共振成像(Task f MRI)技术,分析库欣病(CD)患者脑功能改变及其与精神症状的关系。方法收集2015年11月-2016年11月在解放军总医院内分泌科住院诊断为库欣病的8例患者,并选取21名年龄、性别和受教育程度相匹配的健康人作为对照组,对其进行Zung抑郁自评量表(SDS)、Zung焦虑自评量表(SAS)、正性负性情绪量表(PANAS)和库欣生活质量量表(Cushing QOL)的评估,并行Task f MRI检查。结果与对照组相比,CD组患者均存在抑郁、焦虑状态,正性情绪评分低,而负性情绪评分较高。Task f MRI结果显示,观看正性图片时CD组左侧小脑和右侧中央后回激活程度较对照组减弱,左侧小脑激活程度与16:00点促肾上腺皮质激素(ACTH)水平呈正相关,右侧中央后回激活程度与尿游离皮质醇(UFC)水平呈正相关;观看负性图片时左侧小脑、双侧海马旁回和左侧额下回激活程度较对照组明显减弱,左侧小脑激活程度与0:00点皮质醇水平和焦虑量表评分呈负相关,与UFC水平呈正相关;观看中性图片时左侧小脑和左侧海马旁回激活程度较对照组减弱。结论 CD患者脑功能受损,会出现抑郁、焦虑等精神障碍,通过Task f MRI技术可发现左侧额下回、右侧中央后回、双侧海马旁回和左侧小脑激活程度减弱与之相关。
目的采用静息态脑功能成像探索2型糖尿病患者早期脑部功能损伤。方法对18名2型糖尿病患者及18名健康志愿者进行脑部静息态功能成像及脑部结构像扫描,计算全脑局部一致性(regional homogeneity,ReHo)、低频振幅(amplitude of low frequency fluctuation,ALFF)及双侧海马、后扣带回的功能连接。统计学方法采用成组t检验。结果 2型糖尿病患者较正常对照ReHo减低脑区主要位于右侧丘脑、海马、嗅皮质及左侧壳核,ALFF减低脑区主要位于左侧额中回及右侧缘上回、枕中回。2型糖尿病患者较正常对照海马功能连接主要表现双侧海马之间功能连接的减低,但与其他脑区之间无显著功能连接减低区域;双侧后扣带回与其他脑区之间未见显著功能连接减低区域。结论 2型糖尿病患者早期脑部静息态脑功能主要表现为局部脑区ReHo及ALFF的减低,双侧海马之间功能连接的减低,双侧后扣带回功能连接无显著改变。
Objective To investigate the changes of lateral geniculate body (LGB) in the normal aging brain using quantitative susceptibility mapping (QSM) technique. Methods Magnetic resonance (MR) phase and magnitude images were acquired from enhanced gradient echo T2 star weighted angiography sequence with 16 echoes on 3.0T MR system using the head coil with 32 channels. Morphology Enabled Dipole Inversion (MEDI) method was applied for QSM, and the susceptibility value of LGB was measured by region of interest (ROI) drawn manually on three orthogonal planes. Results LGB of the middle-aged group had a higher susceptibility value (0.16±0.05 ppm) than that of the youth group (0.12±0.05 pprn) and elderly group (0.13±0.03 ppm) (all P〈0.05). Partial correlation analysis demonstrated that there was significantly positive correlation between susceptibility value and age in the youth group (r=0.71, P〈0.05). Conclusion LGB could clearly be identified on QSM in the brain in vivo.
